Here is a video that gives you the steps to painting although I have to say I don't think I would paint this one. Your fireplace surround is very pretty. Maybe you could clean it up a bit (it looks darker at the top of the firebox.) Live with it a few days then decide what you would like to do.

Here's my two cent's worth: clean it up,paint the wall and change out what's on the mantels. This fireplace is really nice. It just needs some punch around it.
I would get some black washi tape and put it on the grout lines. Live with it. If you like it you could darken the grout. If you don't like it, rip the washi tape off. I agree with the others, you may not need to change the actual fireplace. Paint or wall paper the walls and change out the top mantle piece. There are some cool trendy baskets that are natural with black stripes. I think they would look great as accessories. Walls could be BM Revere Pewter or an olive green.

These were some of the brick colors popular in the 50s & 60s.... https://retrorenovation.com/2015/04/13/where-buy-roman-brick/ very Frank Loyd Wright inspired.... the tile rim just doesn't go..... I would paint that for sure to blend more with the masonry or I would rip it off and wood clad it.
I would tape of the mortar so you don't get stain on it, frog tape would be good, and mask off the whole area with plastic around, the floor etc as this can be messy.
