How can I fix a thin concrete patio that is breaking up in places?

House built in the 60"s. The thin concrete patio is breaking up in spots. Can't aff ord to jackhammer whole patio and pour new concrete. Tried patching but, because original concrete was shallow patches don't hold very long. Any ideas? HELP!!!
