CLR works well. Depending on the surface, vinegar or rubbing alcohol will work too.
I just removed some very hard ones that all the other remedies failed. They were in my toilet and caused by my well water. I drained the water from the bowl and sprayed the bowl well with Clorox gel spray. I let that sit for about 3 hours. I flushed and then used a fine grade pumice stone, brand name Pumie, and it came off and left the bowl white with no scratches.
I use white cleaning vinegar. You can buy it in many stores.
