I would stand on the toilet
If your ceiling is the same color as your wall, you can tape a paint brush to the end of a wooden pole (old broom handle, piece of pvc pipe) with duct tape and paint away. Looks like you might need to prime the trim before painting it.
I place my step ladder over the toilet. (2 feet on one side, 2 get on the other) I hope that makes sense.

You do not need to remove the toilet. Stand on it, use a ladder, rent scaffolding. Invite your tall friends over to help you paint the ceiling and trim. Not to fret, you can do this! 🌞
Here is another way - lift the toilet seat up. Drape a towel over the bowl. Grab some scrap wood (sturdy enough to stand on without bowing), set it across the toilet. Gingerly get a step stool, then step on the wood across the toilet seat. Be careful though as this is not the "safest" way to paint, but will do in a pinch.
Lean/tilt a ladder up against wall and/or over toilet with someone securing you for extra safety or use some other way to secure it.

May be hire or buy or borrow a folding platform you could stand on or hire a decorator to do it for you.

I did that and broke the plastic lid. With a ladder you should be able to move it so you have access to the molding above the toilet.
