There needs to be a moisture barrier between the concrete and the hardwoods. Check in with the manufacturer to see what they prefer to be used. Some times it’s a really thin closed cell pad. Others ....not.

Yes....absolutely, but you need a sub-floor padding that is like a thin sheet of dense foam. Your floor will be more quiet and will wear better. It will also feel better walking on it barefoot and weather insulate it in the winter. Check out tutorials on Youtube if you plan on doing it yourself.
