Janice that’s a tough one. Have you tried to spray bleach on it? You’d have to be very careful to not get it on you.
More even use a sponge and gloves to apply. The ceiling being lumpy will
be the issue. A soft brush may help very lightly.

Mans yes the fan will improve things.
There are mode collection kits at the big box stores that you can purchase to see the level of contamination you have. I would do that before I went any further.
If it is just a minor problem and not the dangerous kind, investigate how to remove this. Then seal the ceiling with Kilz and repaint the ceiling. If you do not seal it before painting, any stains will bleed through the paint.
Any of the big box stores should have a mold removal product. Again, here is a link from Home Depot (I do not work for them, but there is one close to us and we use them all the time!)
