How can I show my sister and BIL how much I appreciate them?
My sweet sister and her wonderful husband always include me in Valentines dinner since I became
a widow. How sweet is that? We always have a great time whatever we do. How can
I show them how much I appreciate that they always think of me.

Bring them wine, flowers or a gift certificate to their favorite restaurant. Theater or music tickets may be appreciated too.
a bouquet if you know they appreciate cut flowers, a really GOOD bottle of wine if they enjoy that; tickets to a broadway show or even local little theater if you live in a smaller town; tickets to the symphony for a special production, or season tickets if you know they would appreciate that.
Reservations for a wine/cheese tasting at a local establishment; an offer to BABY SIT if they go out a lot.
A lot will depend on where you live -- a bustling metropolitan area or a sleepy rural community ... . or something as simple as a hand made/hand written card expressing your appreciation ...
Hi Gloria! I am sure they are so thankful that they can do things for you. My sister and BIL are that way too. I occasionally will get them a gift card for dinner out at their favorite places .... for instance, Cracker Barrel or Red Lobster. My sister has to get chemotherapy once a month and she is so hungry after it and she loves Steak and Shake, so I get them a gift card for S&S and keep adding to the balance. I also will give them a free tank of gas and I will also pump it for them. How about going to the grocery store for them at different times! I know what they like to eat and I will buy them shrimp, frozen bread dough, ice cream.....things of that nature. Not only does it help them out greatly, but it shows them how much you appreciate all that they do for you. Just being there is a big thing to me! There are many other things you can do too:

Take them out to dinner so your sister doesn't have to cook and clean up afterwards. If that isn't an option have their favorite meal delivered to their house. Again no cooking for your sister. And if nothing else buy a gift card to their favorite place to eat.
A simple note or card is nice. Just telling them that you appreciate them is all that is needed. An unexpected handmade craft gift also helps. You don't need to spend a lot of money to get your point across.
How about bring them flowers when you visit. I would love that as a host.
