You don't. You break it down with a Sledgehammer, Remove mostly all of it that you can, and Pour Concrete to create 6 foot Slabs between 4×4×48s.
A photo would help. How bad is it? Is it cracked? If it's just spalling, flaking, and level it can be resurfaced. If it's out of level, severely cracked, separating it would need to be removed and a new one poured.

Sometimes they fall apart from water intrusion and the spalling is fatal, through efflorescence.
