I have a small particle board plaque with my dogs name on it but I want to place it outside on my gate. How can I waterproof it from the rain?

I don’t think it will last forever, because you’ll also have UV damage over the years, but I’d put 3 full coats of “Spar Marine” Polyurethane on it.
Something made for boats, I'd say, like a spar urethane or marine urethane.

Marine strength Poly will do the trick but don't forget to coat both sides.
You can apply acrylic sealant in every edges of the board as well as to the area which will be most probable to be exposed in moisture.
