if the foam has broken down in addition to the upholstery tearing or being worn out, you’ll have to go to a foam store & cut the foam to fit.
I’m guessing you mean the driver’s & front passenger’s seat, yes?
If they are vinyl, I’ve repaired small rips/tears with an automotive vinyl patching kit, after I’ve inserted foam in the failed areas.
If the seats are fabric, I would stabilize the damage and then put car seat covers on them, like slip covers.

Stretch covers you buy at an auto supply store.
Do you need new outside covers only? This is called recovering.
Or, does the entire inside stuffing and the outside covering need to be replaced?
This is called reupholstery.
It may be possible to sew new elasticized slipcovers for your seats. The difficulty of the project will depend on creating a pattern, the style of the seats, and how they are attached to the vehicle floor. For best results in having the new covers stay attached to the seats without popping off, it is recommended to remove the seats completely first.
Reupholstery jobs are way more complicated. It is important to use the foam having the right thickness and density. These jobs require specialized tools, an industrial sewing machine, and knowledge on how to use them properly. There is a reason why professional reupholsters charge what they do for vehicle seats.
