Prevention: dress shields are sold specifically for this purpose. A Google search of "dress shields" yields many sources to purchase them. Or, you might prefer to make them, which is more economical. A half-yard of flannel fabric in a solid color close to your skin tone will yield several, depending on the size you choose to cut. They can be from 3X3 inches to 7X7 inches, depending on your build and preference. They are pinned to your garment with safety pins, and washable. Hemming will prevent fraying, or a product called Fray Check is useful for this.
Stain removal: To remove sweat from clothing, soak the item in a mixture of cold water and white vinegar for between 20 and 30 minutes. Then, run cold water over the soiled area, and gently rub the area with your fingers to loosen the stain. Then, wash the clothing like you normally would — in a load with like colors. Chlorine bleach is not recommended.

Use an antiperspirant instead, and use Stain Devil for perspiration stains on your old shirts. If you find an antiperspirant doesn't work for you, maybe you could ask your Doctor what he would recommend for the problem. Hope that helps.
