You need a consult with a foundation expert as well as a soils engineer. This is not a Homeowner DIY, you need licensed professionals for this job.

I agree with Naomie. The only thing I’ll add is tell you about how it is fixed. A company will come in with hollow core bits. They are fairly large (3-4 inches). They’ll drill a series of holes and insert a pressure hose into them. The hose pumps in a fairly loose mix of concrete which raises the slab up even with the others.
So, I'm in Michigan, would you have any idea of aprox what this would cost?
