Is it going to be covered? There's a patch product SIKA that might work but it will be a different color. Another option is to chisel out and re-pour by mixing a ready mix product like Sakrete that's a "just add water" solution.
A cracked slab could mean you have some foundation problems. My suggestion would be to call in an expert just to make sure. Most of them will come out for free to give you quote.

There must be an underlying cause for this, like a shifting water table, I would locate the cause and go from there, it may require professional help rather than a band-aid approach.
