The only way to do this is with good old muscles and sweat! Get yourself some eye protection and a big hammer. Remove your sink beforehand (making sure the water is turned off) and then start breaking up the tile and concrete! You could try to use a pry bar to lift the countertops but be warned - they will be heavy and prying them up could damage your cabinets. If you're trying to save the cabinets use caution separating them from the countertops.

What are you going to replace it with? If you plan to demo removing the concrete and all, I would rent a hammer drill. You will need goggles, good masks, gloves, various pry bars and tin spits, a big trash can to remove debri, and plastic sheeting, even masonite boards to protect floors.
