I would divide it into sections. Maybe some raised flower beds in one area and vegetables in another. Add a patio area with pavers and a fire pit. Draw it out on paper and do one section at a time.

I like Ann's answer and maybe a wildflower/pollinator garden at the back.
Just as Rymea mentioned, I would turn the back area into a native garden that require little to no care and can look beautiful. Just check into your local natives which also will provide food and shelter for local fauna which can be quite fun!
I would work on getting that grass back in good shape and fill in the bare areas. I would mix Weed n Feed and some grass seed in a lawn spreader and treat the whole thing. If needed, water the lawn daily to get the new seed to grow.
