Everyone says that you don't have to sand before you paint, but I have had a bad experience with it when the piece was heavily varnished . You definitely have to clean well first. If there is a flat finish , go for it . If the wood has been polyurethaned, I'd sand lightly anyway and wipe down then paint .

Not usually. Cleaning the surface is very important. I would lightly sand if the surface is shiny or slick.
Sanding is not necessary when using chalk paint.
