1) the installer should be specially trained by the manufacturer
2) stick with Rinnai or Navien as these have time tested track records
3) you must have adequate natural gas supply. Sometimes it will require a larger gas supply line for the whole house so other appliances don't get starved for pressure.
4) some require a special vent pipe
5) some have a recirculating pump. It is more expensive, but I highly recommend it since it will save you energy later
6) flow is important. A trickle of water will not activate the burner on your water heater. To get hot water, you usually need to turn on the faucet full blast for a few seconds then lower the flow.
Knowing what to expect and how to maintain these is critical. I'm a huge advocate of tankless water heaters, but you need to understand their limitations, have realistic expectations and they must be installed properly to perform optimally.
