The only real way to fix a "slab" is to have it ripped out entirely and totally replaced. You could consider just building a wood deck right over top of it.
It sounds like there may be moisture getting underneath and until you fix that problem the other problems will never end. Remove and replace if necessary. If you place anything over top, it will eventually move and heave too. If you do decide to redo it then be sure to place a good gravel base underneath before you start pouring the concrete.
Often cracking concrete is caused by tree roots. Everything I’ve read is not encouraging. Supposedly if you cut them out or grind them out they’ll come back.
