It is an insect larvae called a grape gull midge. They can be different shapes and sizes and are usually harmless. When the larvae hatch they fall to the ground to mature. Removing the gulls and destroying can help to slow bug population however.
There is an oil from India, called Neen or Neer oil, available at HD or WMart. It is a natural product, and can be sprayed on the whole plant with no harm to man or beast.... Just had to spray my peach tree and the remaining fruit trees in my backyard, as the nasty little critters are taking over....
I use Ivory dish soap 5 drops to quart of water to get rid of all sorts of pest, even bull ants. Will not hurt the plant.

Then squish them dead.
Orange Larvae will hatch from these swellings. Destroy any larvae you find before they to underground to return later as Midges.

It is actually called Neem oil. Please, if you use neem oil, follow directions carefully. Please DO NOT spray or use when bees and other pollinators are active, only on windless evenings or just before sunrise. Though it is used in organic gardening, it Will Kill Bees!

They are vine buds. Expect them to continue leafing and growing next season.
