Add a larger mantle and some type of wall accent above it to the ceiling such as shiplap. Here's a photo to show you:

You should be able to take a chisel to the fluted trim and rosettes on the face of your fireplace. If they're just nailed, they'll come off easily. However, if they're glued and nailed, it may take a little more work...and possibly some wood putty and sanding. But it can be done! Then, you can repaint or add new trim, tile, stone, whatever.
you should be bake to carefully remove those pieces you don't like then sand and then repaint. You can also add a back splash above the mantle to take the focus off the white.
Can I just fill in the spaces with wood putty and then sand it?? might be easier.
Wish my fireplace was as pretty as yours. Mine is brick with a cement mantle and brick raised brick hearth and a long cement slab you can sit on. Hate it and would like to paint but once you paint, that's it. Would never buy another house with a fireplace in the LR...it's dirty and sucks all of the heat up the chimney. Purchased electric logs a few years ago instead of burning wood or Duraflame...less mess.
can you make a long cushion to sit on to hide the cement?? Our fireplace is gas.. it was a wood burning but we changed it because of the mess..
Hi Deb, what if you put an oversize grey mantel on it and some slate for the hearth, you could do a large picture, faux weathered wood paneling or clock above the mantel
