This looks like it might be a prefab insert...more like a heater.
Keep in mind that you have make sure flammable material (like wood) are a certain number of inches from the firebox on any fireplace source of flame. Check your local ordinances or google information on building fireplace surrounds before you start.
If you are very skilled in woodworking such as cutting molding, it will be just a matter of choice to pick out a design and go with it.
However, you can make an accent wall by going to Board and Batten and just enclosing this into the design. It will take some skill in measuring to make sure you get the Board and Batten evenly spaced.
All of this can be investigated online as there are many choices.
Tile over and Use Brackets , if required to add a thicker Shelf/Mantle
It looks like those are just two boards that have had their corners rounded. I'd remove them and install a larger premade mantle.
I have a faux brick and was gas fireplace old style, washed the faux tile with TSP and then rinsed off. Next put primer white on all the faux bricks. Then final coat white paint. It really looks better than it was before. A weekend project.

You could build a box with wood that could be mounted to cover right over this. Leave it plain or use moldings to give it whatever style you’re after.
I would be tempted to use a couple of corbels to hold up a wider mantel and maybe some narrow fluted boards running down each side of the fireplace. We made this one and I loved it.
