What can I do with this?
I thought I might find something good under all the black & then hot pink paint, but all I found was unsealed MDF. Should I prime & paint or just spray paint?

I would prime before painting it
yes, definitely prime and use fine sand paper or steel wool before painting

You could apply a wood vaneer, tile, faux marble vaneer, paint a stone vaneer, use brown paper bags for a faux leather look, decopage book pages on it, copy a quilt pattern with fabric scraps or paper; my goodness, paint or not? The possibilities are ENDLESS!
I had a small table I painted black. Then I decoupaged printed burlap on the top. Then I put a little stain on the burlap to age it. You can put a sealer if you put anything wet on it.
Prime first. MDF needs to be sealed. Paint whatever color you want. If it is a dark color you can polyurethane it. If light color use polycrylic.
