We replaced our Vanity, sink, toilet and the bath and sink hardware ourselves. We have plans to do the flooring ourselves also. Don't know what we are going to do with the walls yet. It has been done in increments due to leaks under sink and dripping faucet, the other bathroom toilet no longer had the innards available to replace, so we just replaced both. I hope you can do yours in a more normal sequence, but it is still doable for us.

Cost effective depends on how much work you do yourself v. hiring out, cost of materials chosen and how much of a remodel do you want to do. Strip to the studs and start over or just change a few things up a bit and more of a face-lift? Anything is possible with a reasonable budget and some basic DIY skills. Savvy shopping skills really help.
keep the same floor plan, new fixtures where the old were. The flooring goes under the the toilet, with the correct size hole for the waste pipe of the toilet. Last time I changed a toilet was 3 years back, had to put 2 wax rings down to get it sealed.
Remodeling bathrooms come in all sizes and costs. If you have some skills, I E. Like changing a light fixture, painting, maybe a little bit of carpentry And willing to do it yourself, at least some of it you will save money. Totally gutting a bathroom will cost a lot. If the tile and tub area are in good shape maybe it just needs a face lift.
