That's not just a fan it's an entire ceiling system that is about 2 feet taller in center
You might be able to install a hugger fan. Make sure it's installed on a ceiling joist. I agree with Kelli, the photo has a raised ceiling. There are several versions of low profile ceiling fans.
Another option would be a fan that is encased in the light, like this:

The picture is of a tray ceiling. If you have a floor above your kitchen, you won't be able to install one. If it's just attic, it's probably possible, but will cost quite a bit. I suggest you look into hugger fans where the motor installs directly on the ceiling. This is just a quick search to show you what I mean and give you a start. Be sure to check how high they are to make sure they fit with your low ceiling. I don't know if a light is such a good idea.

I agree the photo is a tray ceiling. A photo of yours would help. But more likely it wouldn't work unless your ceiling is raised.
