There is a leveling agent you can get for Concrete but I don't know if it is viable for outdoor use...???? check it out.
Be careful what toppings you put on your patio because some of them get very hot and peel.
If the pooling is gradual, you might want to look into putting down some pavers and making sure you keep putting a level on them as you go by using more mortar in low places.

You should clean the concrete first, with muriatic acid solution if it’s been pooling for years.
Then apply a concrete bonding agent and then the concrete leveling topping.
You may need to screed it, which is typically a 2 person job.

If solid concrete just use a concrete resurfacing compound.

If the patio is concrete, I agree about resurfacing and also leveling if that is a problem. Stripping and resurfacing the top is sometimes all that is needed to prevent the water pooling. Be sure the patio is level. Hope this helps
If your patio is a paver patio, you might have to bring in a professional to have your patio leveled properly.
