How you do it will depend on how deep and large the cement plug is. Are you planning to do this yourself? Cement gets heavy quickly as the volume increases. What is the post made of? You need to dig it up or pill it out with a truck and chain.
Yes you will need to dig out the old concrete plug and pour a new one to set the post into. You can get a bag of Ready-Mix, so you just have to add water!
