Regrade any area to slope away from any structure. You might consider regarding the yard too and possibly installing French drains. I have 3 on my small piece of property. Works like a charm.
You can go on-line to do a "FRENCH DRAIN" . They have step by step instructions.

Grade the land away from the home. Lay Shingle One foot deep around the property and or Fix Guttering and Downpipes that terminate in Water butts with taps on so in dry weather you can water plants etc.
I would install a french drain that releases water into the storm drains.
