I used to add a ruffle around the bottom of t-shirts and sweat shirts for a dress. You can decorate or leave plain.
If it is a longer t-shirt that goes just above the knees, you could tie dye it and make it into a t-shirt dress.
When my girls were little, I would cut out the sleeves so that there was just a sleeveless or cap sleeve. Then I would add to the bottom with fabric from another clothing item they had outgrown, like a skirt or lower part of a tshirt, add buttons or some other trim. Patches, appliques and even fabric marker can make it fun.

Lay your child's simplest dress on a sheet of paper and cut around the shape allowing for seams, this should give you a pattern you could use many times as a guide. The most important bit is to get the shoulders right and the armholes and neck. All the rest of the dress can be variable. Best wishes. Make up one from the pattern and adjust the pattern if required for a better fit.

Because t-shirts are knits, when the fabric is cut, the edges will be raw, and will stretch and fray very easily. Without binding and reinforcing these edges first, once washed, the integrity and shape of the new 'dress' will be warped.
This project is worth the time and effort only if you bind the raw edges, preferably with a serger.
Cut out the arms and neck of the t-shrt,try it on the child to see if you have to do any more cutting or adjusting to make a comfortable fit. After doing that, you'll have to bind the raw edges or they will distort the sundress or fray out. Pockets can be made out of the sleeves. As an accessory, the neck can be converted into a headband. It can be fancied up be a ruffle on the bottom and around the arms.
