Definitely cotton !!! Less likely to cause any irritation .
for the 'FILL' .. 'Fiber Fill' like is in any pillow ... and 100% cotton cover ... but babies shouldn't have pillows ... too much risk of smothering ... if for decoration, then something that's not 'fluffy', but firm and not going to present a safety hazard.
Satin is so soft! Make sure you pre wash the fabric for softness and to get irritants out. :)
