It's best to remove the old and start with a clean slate. It will also give you a chance to make sure that there is no wood rot or mold. Either way, you still have to remove the toilet to do a proper job. If you go on top, you'll need an extra thick wax ring for the toilet and it could be difficult to bolt it back down. The vanity can be removed as well. If you do remove the floor tile, and if it was done right, it's under the vanity, already. Sorry, but removing is what should be done.
Unfortunately you can't tile over tile, you'll have to remove the old stuff first and get the surface prepped and ready for new stuff.

Your floor will be extremely heavy so it isn't recommended. You should remove the old floor first. If you can't, you will need a new subflooring above the existing tile then tile over that.
