Where? Glass door or in wall. They sell kits for both.
Truthfully, it would be easier to purchase a doggie door to put in a slider, wall or door. They come with templates for the cut out, frames, the doors they go in and out, every thing that you need. We use a double doored one made for weather extremes (live in Minnesota and get lots of heat, humidity and freezing cold without air leakage. We have four dogs constantly going in and out and five years later still works like a champ.

Hack? As in hack a hole in your door?
Then, use a saber saw, as a sawz-all is too hard to make a straight clean cut.
Or as in hack, like steal the concept of a pre-made one?
Then, if your time is worth 10¢ an hour, you’ll have to find hinges that self close, gasket/weatherstripping and cut a piece of thick aluminum or a UV stable plastic.
Hollow core doors can’t handle this kind of pet door without re-enforcing the hole.
