How can I repair a painted ceiling that has mold growing under it?

Just guessing here but how is the ventilation in the bathroom? Do you have a fan? Do you have a window?
Could be the steam from the shower isn't able to escape the room??
It sounds like it would be a type of mold growing on the ceiling. Do you have good ventilation in there?
Lack of ventilation in the room. Exhaust fan? Window? Leave the door opening when showering. Even a portable fan will work. Just use a vinegar water solution to wash the ceiling. It's not mold, just dust and dirt.
You have a moisture problem. Do you run or have any fresh air? Do you smoke? Nicotine will do this. If you have gas forced-air heating, then it's from that - a residue is left on the walls. You should be able to just wash them off. To prevent them you need an exhaust vent, or one that does a better job - and make sure everyone turns it on when they bathe!
I would say remove it, clean beams and install new ceiling

I would scrape first. Then kill the spores with white vinegar. Now you can prime and paint. Be sure to use a stain blocking primer and mold inhibitor paint. You sound like you need a fan added to pull the moisture out. We did a small ceiling fan in the bathroom.
You have to treat the mold before you repaint it. You have moisture or a leak coming from somewhere that you need to find and repair first so the mold won't continue to grow and spread and mold is not healthy for you and your family. Once the moisture issue is dealt with you can clean it, use Kilz to get the stain out and then repaint.
Find the leak first and then go from there
There are several ways to get rid of mold, but most importantly, you need to address the root cause to keep it from returning.
