Is it fastened to the bed or to the wall. Sometimes the tilting comes from the bolts holding it to the bed frame. The can become loose. If it is fastened to the wall and come loose then use angle brackets or a wooden cleat to hold it in place. Hope this helps or gives some idea of what to look for.
Sounds like the bottom screws are loose where attached to frame. Make sure you have at least 2 screws per leg.
Have you tried tightening it sometimes it just gets loose.
Make sure the sideboards are properly attached to the headboard. Usually there is a metal bracket with two teeth that fit downward into a slot. If they are not properly seated the head board is not properly attached. If there are bolts that hold the headboard to the bed tighten them also.
