Sand it down, fill the holes, sand again, clean thouroughly, sand again, prime, sand , paint- its a lot of work but it will make all the difference!
You must address the problem, before you go about trying for a solution. If your door gets a lot of exposure to direct sunlight, chances are the paint used does not have UV protection. If it does not get a lot of direct sunlight, chances are a good primer was not used before the final coat, or the paint was of poor quality.
Home Depot and Lowes sells a special "door paint". It may however not offer you the color choices you prefer as it is premixed. In which case, sand, fill, sand, clean, prime and use an exterior UV protection paint.
True Value sells door paint, I think is made by Rustoleum.
Thoroughly sand and clean your door before painting. Preparation is everything if you want a good finish.
Just saw a Benjamin Moore paint commercial for their paint that is supposed to address these issues. Might be something to look into. I have never used that paint before myself.

Sand or scrape level the raised areas of paint and any parts of the moldings where the paint is peeling, dig the paint and crud out of the gaps in the cross-grain joints, fill them with wood putty, sand the putty level, put a coat of primer over all the areas where you have removed paint, then repaint the entire doors.
