Head over to the fabric store and find some pretty flannel to sew on as "backing" - nice and cozy!
Without knowing how big the scarf is and what kind of fabric and how large you want the throw..... it's hard to give you a good answer. More information would really help!!

What size is the scarf?
Just sew a blanket to it..........You could always add a trim to edge.........
As far as using it as one side to a 2- sided blanket, if the fabric content of the backing that you sew the scarf to, and the scarf, are not the same fabric content then when you wash the "blanket" any fabric content that is shrink prone will shrink. This will cause the "blanket" to no longer have a firm flat blocked shape.
Also, depending on how open the weave of the scarf knit is, it will be very difficult to sew the backing on so the seams are well anchored, and so that the stitching does not pull out.
