I've seen dozens of pool noodle hacks, but, never one for a dog collar. Wouldn't it be too bulky?
Get the smallest diameter noodles you can find. Cut to the length you need for your dog. Use 2 pieces of plasticized wire (in the craft dept. or store) to run through the noodle. Attach one end of each wire to one end of a buckle so you can close the dog collar around the dog’s neck. I don’t know how safe or secure this would be, but I’m sure you have a design in mind. I have seen these used instead of a cone to keep your dog from licking a wound. They were cut in to sections then placed on a thin collar, longer than normal. However, I don’t think it would help if the wound was on a paw.

Take a pool noodle and cut it into sections, they don't have to be very long, just long enough to allow the dog to have flexibility and move freely, Thread the existing thin collar through the noodles and fasten it together.
