you could till it with a garden tiller and take a rake and smooth it out.
If you can hire out that would be the easiest. I would have the entire area tilled over, remove as many weeds as possible and regrade properly. Then decide what you wish to plant.
That would of course destroy the lawn. I would use a roller after a heavy rain. Then use a core areator to loosen the soil.
Tilling doesn't always work if it is really bad. Rolling in the spring is good.

If it turf grass you can sand it to start getting it evened back out. Avoid mowing when the ground is soft. Check for moles/voles/chipmunks.
