If it is twisted properly into place, it shouldn't. Is it the proper length? Do you have a friend to help come lend a hand (if applicable, not everyone is comfortable yet ith people in their home.)

The weight of the fabric hanging from the rod plus the force of tugging the curtain open and closed might together exceed what your particular tension rod is rated/designed for.
Do both ends of the rod actually twist, as Naomie refers to, or does the rod just 'push' into place? Also, are the ends of the rod rubberized?
This sounds like an easy fix!
Take you tension rod down and clean each end with rubbing alcohol and also the area where the tension rod meets the walls. This will clean the surfaces of any soap or shower film that maybe contributing to it slipping. Here is a link on how to adjust your shower adjustable rod:

It sounds like it needs adjusting. Usually, just a couple of turns takes care of the problem.
Is it old? Sometimes the spring just wears out.
I have tried everything on mine and I have finally given up and put permanent holders for the rod. The new style rod (the one you twist) isn't true spring tension and will not hold up like the old style does. I went with the lightest weight single curtain. Cleaned, even used gorilla tape to try and hold in place. But in a few days, there it is in the tub again.
I used the same thing that my clothes rods use. It is a small plastic U shaped cup that the rods sits in.

Hi Judith, you may have too much weight on it, try grabbing another bar an use one for inside the tub and one outside the tub.
I bought some conduit and sprayed the metal with silver Rustoleum. Put flanges on the end and attached to the wall. Now it is permanent and no worries.
