just get padding from a fabric store and cut it to size. Get material and cut it about 4 inches bigger than the top on each side. Staple with a heavy duty staple gun on the bottom of the lid.
Just make a long cushion with a piece of foam or sew 3 cushion together.
I would just place a foam piece that is covered with fabric on top, and not fasten it. These are sold by the size in catalogs or you can make your own. Walmart sells foam an fabric. In the alternative, I have a folded pretty quit and placed on top of mine, (which serves as a stepping stool for the dog to get to the bed!) It covers it totally - and when I redecorate, it is easily changed to match the décor.
If you want it to look upholstered, cut plywood to match top, add foam for cushioning, wrap in batting and staple, wrap in fabric and staple, screw through bottom on lid to attach, countersink and fill holes.
