Keep an apple slice in the brown sugar, that will keep it soft. Be sure to use an air tight container.
Try putting a piece of apple in the bag to moisten it up. My mom told me that years ago. Air has gotten in and dried it out. It will still moisten up when you use it in cooking. You may have a little pin hole in your bag.
My grandmother always put a piece of bread in with hers.

I have always heard that a half an apple in a container with the sugar will soften it up. I keep mine in a plastic flour canister with a clay sugar softener (Google: search> "clay sugar softener") The clay is un-glazed, you soak it in cold water for about 15 min... dry it off and put it in the canister with the brown sugar (I leave mine in the box or bag) any time I open the canister..I "recharge" the clay piece, so it does not dry out completely)
either just cover it and only for a few minutes
