Paint or stick on Fabric - Add lace around edge or any other trim. Use centre to display a Photo or Gift or Message - Use as template for Cushion? Fold in half and make as Card? Add Mirror Tile or Photo's...............
I would suggest either covering with fabric (for the simpler/quicker project) or hand painting a base color then gluing pretty beads over the entire heart-or a design of beads. Similar to a Fabrege Egg.

Cover it with fake flower heads and add some pretty broaches or pearl pins for accents.
How about making copies of family pictures and modge podge them to the heart. Decorate with small flowers.

Staple pretty fabric to the cardboard heart, and attach ribbon for a hanger. Cute bulletin board!

Cover with a collage of photos associated with the family (Mothers Day) or for the birthday significant events in the birthday boy/girls life. Then make it a wall feature by adding a light in the back (battery powered so all the cables are neatly hidden) or a string of lights battery powered around edge to lightup the photos. Cover the cardboard with aluminum foil, then bake cakes, let them cool, then cut them into heart shapes almost the same size as the cardboard. Place the heart shaped cakes (which may consist of multiple pieces, depending on how you made the heart) on the aluminum-covered cardboard and Frost them. Suggestion: red velvet cake. Easy, quick, and inexpensive

I wonder if Unicorn Spit would work on cardboard. U-Spit, acrylic craft paints, and Mod Podge seem to be the best alternatives to spray paint.
You could always paint them with craft paint and add decorations using a hot glue gun.
