Hello Audrey,
Knock the pot off or if difficult go around the outside of the pot with Spatular or flat blade knife. Hold by the trunk to try to avoid leaf loss, but don't over worry, they will soon grow back. place into a larger pot you have already added soil to and back fill with rest of soil. Water and leave to settle and drain. You are done! More to new home. Enjoy!

You're in luck, this is one of the best times to repot a jade plant. They are succulents and like well-aerated soil. Here's a link with great instructions:

No differently than any other plant. Jade plants loose leaves easily so you have to be gentle. The best way to handle is at the base of the plant where it meets the soil and handle as little as possible to pop into your already prepared new pot. Don't let go of the base until you have sufficient soil around the roots to hold it upright then fill the rest of the way up. Get a helper if you need. It is only because the leaves are so susceptible to breakage.

I'm assuming you want to repot b/c it is outgrowing what it is currently in? Just take all the roots and add it to a bigger pot. Place the current soil and roots in the new and bigger pot and add some new soil around the edge. Water and place back where it was!
